Howz works with NHS Trust to help protect people with dementia and their carers during the Coronavirus outbreak
Surrey and Borders Partnership NHS Foundation Trust is delighted to announce that referrals are now being accepted for a new, free of charge remote monitoring service to support people with dementia and their carers across Surrey during the Coronavirus outbreak.
The TIHM (Technology Integrated Health Management) Monitoring Service combines easy to use digital devices installed in homes with a clinically-led Monitoring Team to remotely monitor the health and wellbeing of both people with dementia and their carers. It will reassure some of our most vulnerable citizens during this challenging period and help to identify early signs of ill-health so that prompt treatment and support can be provided.
The TIHM Monitoring Service is offered by Surrey and Borders Partnership and smart home monitoring provider, Howz, in partnership with Surrey Heartlands Care and Partnership and Surrey County Council. It is based on the award-wining TIHM for dementia system.
The Service is available to 1,000 homes across Surrey.
People who have a diagnosis of dementia, who are aged 70 years and over, and who live at home in Surrey
They must also have a carer, who can be a partner or relative, living at the same address. The carer can be of any age
How will the Service support people?
People will receive a combination of easy to use digital devices to regularly monitor temperature, oxygen saturation and pulse, and changes in activity levels in the home. They will also be asked to regularly compete a short health questionnaire
The health data will be analysed using data analytics and machine learning
Concerns identified by the technology will be flagged on a digital dashboard and followed up by the clinically-led Monitoring Team which has support from GPs and Social Care teams. The Team is contactable from 8am to 8pm, seven days a week
The Monitoring Team will, where necessary, arrange for people to receive coronavirus testing and will also liaise with 111 and hospital services
